Housing & Community Support
The Housing and Community Support Program is a day program for male youth ages 16-24 who are homeless or at risk of homelessness and are already connected to the Shelter or other programs offered by Turning Point Youth Services. The program is operated as a component of the Shelter and is located at 95 Wellesley Street East.
The Housing and Community Support program uses a client focused, strengths based approach. Interventions are relationship based and focus on narrative, solution focused and cognitive behavioural approaches. The Housing and Community Support program is responsible for supporting the Action Plans or Plan of Care developed for clients by either the Shelter or other agency programs. The program works with youth to seek affordable housing, establish connections with landlords, access services within the community, develop independent living skills, relocate to more suitable, longer-term living situations, and to provide support once living independently.
The Housing and Community Support Program works in collaboration with community services and organizations to coordinate services and ensure that the needs of the youth are being met.
Youth are able to reconnect with the program for follow-up as they feel the need and the program will connect with the youth by phone once per month for the first three months to check in and provide support as required.
Project Going Home
Project Going Home supports individuals, 16 and older, and families, who are homeless or at risk of homelessness within the City of Toronto, to reconnect with or establish support systems within their home community. Referrals to the program come from individuals and families, or from other services within the community such as shelters and hostels. The program offices are located at 95 Wellesley Street East.
Project Going Home uses a client focused, strengths based approach to develop an individualized plan for each client that best meets their needs. Project Going Home assists homeless individuals and families to return to their home community if it is clearly deemed to be in their best interest to do so. The program provides the following services:- Accommodation - the program seeks and/or verifies that the individual or family will have accommodations within their home community
- Connections / Referrals - referrals are made to local supports and services that will continue to support the individual or family upon return to their community
- Travel Support - eligible clients will receive minimal travel support in the form of toiletries, and bag lunches
- Transportation - transportation is provided, as necessary, to facilitate clients return to their home community
Project Going Home works in collaboration with relevant community services and agencies to ensure that all provisions are in place for a smooth transition for the client back to their home community.
Clients who return to their home community receive follow-up support to assist in their successful reintegration. Program staff will make telephone contact with the client(s) at least 1 week, 1 month and 3 months following their return to their home community, in order to check in and ensure that their needs continue to be met.
